Using this website

This website provides information about Arlington Dental Associates, dental care and visiting the office. It does not offer online accounts, appointment-request forms, payment processing or document uploads. To discuss an appointment or how to provide records, contact the office.

Technical information

The website is hosted by GoDaddy. Hosting services process technical information when a browser requests pages and files. The practice’s account is configured to remove the previous month’s archived access logs at the end of each month. This setting does not establish retention for all provider-managed logs.

Cookies, analytics and outside resources

The website does not add advertising trackers or analytics, and its fonts and images are hosted with the site. Links to outside educational resources take you to websites with their own privacy practices. The website’s own code does not set cookies. Google Maps loads automatically when you visit the Home or Contact page. The embedded map and Google directions or reviews links connect your browser to Google, which may receive technical information such as your IP address, browser details and the website’s origin and apply its own privacy practices. Separate services provided by the host may have their own data handling practices.

Who this notice covers and how to reach us

This notice concerns health information in the practice’s care, including dental records and billing information. The separate website privacy section explains the informational website. The website does not accept records or privacy requests online.

Care, payment and practice operations

We may use and disclose health information as permitted for treatment, payment and health care operations. For example, we may provide relevant dental findings to a treating specialist, send procedure information to an insurer for payment, or review records to improve care and manage the practice.

Service providers working on our behalf must protect information under the applicable privacy requirements and agreements. Disclosures are limited as the law requires. If a recipient is not subject to the same privacy rules, information it receives may be shared again and may no longer have HIPAA protection.

Other disclosures allowed or required by law

Where the applicable legal conditions are met, information may be disclosed for public-health reporting, medication or product safety, suspected abuse or neglect, prevention of serious threats, health oversight, workers’ compensation, specified law-enforcement or government activities, organ donation, or a coroner, medical examiner or funeral director. Court orders, subpoenas and other legal requests require the applicable safeguards; a request alone does not remove those protections.

We also disclose information when a law requires it, including authorized oversight of privacy compliance. More protective state or federal rules take priority. We do not use your health information for research or fundraising under this notice.

Your choices and written permission

Tell us your preferences about sharing relevant information with people involved in your care or payment, including family members, and for disaster assistance. When you cannot express a preference, limited sharing may be permitted using professional judgment about your interests.

Uses requiring written authorization include most marketing, sale of health information and most uses or disclosures of psychotherapy notes. Other uses not described in this notice require your written authorization. You can withdraw an authorization in writing, except to the extent we have already acted on it.

Specially protected records

If we receive substance use disorder records protected by 42 CFR Part 2, those records and testimony about their contents cannot be used or disclosed in proceedings against you without your written consent or the required court order after notice and an opportunity to be heard. An order authorizing disclosure must also be accompanied by a subpoena or other legal requirement compelling disclosure. These protections apply to civil, criminal, administrative and legislative proceedings.

Texas law can provide additional protections for sensitive records. When a more protective rule applies, we follow its permission requirements and exceptions. For example, Texas restricts electronic disclosure outside permitted purposes such as treatment, payment and health care operations unless an applicable authorization or legal exception permits it.

Access to records and corrections

Contact Dr. Chei to request inspection or a paper or electronic copy of records. Requests may be subject to lawful exceptions and reasonable, permitted copying or delivery charges; retrieval fees are not charged. For a written request for electronic records that our electronic system can provide, Texas law generally requires delivery within 15 business days. Other access requests follow the applicable deadlines.

You may request correction of inaccurate or incomplete information. We generally act within 60 days. If a permitted extension is needed, we explain the delay and completion date in writing. If we deny a correction, we explain why and how you can submit a statement of disagreement.

Private communications and sharing restrictions

Ask Dr. Chei for a reasonable alternative contact method or mailing address. You may also request limits on information used or shared for treatment, payment or operations. We do not have to accept every restriction, but must accept a qualifying request to withhold information from your health plan for payment or operations when you paid for that item or service in full yourself, unless disclosure is required by law.

Disclosure history, representatives and copies

You may ask Dr. Chei for an accounting of certain disclosures during the preceding six years. Disclosures for treatment, payment or operations and other legally excluded categories are not included. One accounting within a 12-month period is free; we explain any permitted charge for another before proceeding.

A legally authorized representative can exercise rights for you once we verify that authority. You can request a paper copy of this notice even if you previously chose an electronic copy.

Our responsibilities and changes to this notice

We must protect the privacy of health information, provide notice of our duties and practices, follow the notice currently in effect, and notify affected people after a breach of unsecured protected health information when required by law.

We may revise this notice and apply a permitted change to information already held. The revised notice will identify its effective date and be available in the office, on the website and upon request.
